We will walk you through every step of the way to submit to health insurance by using what is called a superbill for reimbursement of an out of network provider.

Psychiatric Diagnostic Assessment
I will ask questions to learn about your past and experiences that have shaped who you are. This will lead to what your life looks like currently and the symptoms you are experiencing. After I learn more about your past and current present symptoms impacting your life. We will discuss your goals and expectations moving forward. I will discuss medications I would recommend to best help you.
Other resources may also be discussed that may benefit you to live your best life!

Annual Mental Wellness Exam
This visit will provide you with some education moving forward in your life in case of an unfortunate life circumstance presents itself causing situational off balance of your mental status equilibrium. The goal of this visit is for you to feel more confident and comfortable in the future to ask for help if you need. You will leave with a better understanding of Psychiatry and benefit it may bring you.

We will discuss medications, symptoms you’re experiencing, medication side effects and medication changes/recommendations.

GeneSight Pharmocogenomic Visit
GeneSight Psychotropic is a pharmacogenomic test that analyzes clinically important genetic variations in your DNA. The results of the test can inform your doctor about genes that may impact how you metabolize or respond to certain medications commonly prescribed to treat depression, anxiety, ADHD, and other mental health conditions. This visit (15 min) will include a brief psychiatric history and previous medications tried. We will discuss the benefits GeneSight testing can do for you. If appropriate, the GeneSight test can be mailed right to your front door or in-office collection would also be available to you. A follow-up visit (15 min) to discuss results is included. (Please note if you would like prescriptions ordered and adjustments to your medications a Medication Management Appointment would be required).

To Find Out How Much Your Health Insurance Company Will Reimburse, consult your health insurance brochure, and/or:
Step One: Call your health insurance company, either before or after your first visit, and have the following conversation:
- Tell the person with whom you are speaking that you are seeing an “out-of-network” provider.
- Ask how much you will be reimbursed for CPT (Current Procedural Terminology) Code 90792 or 99205 for psychiatric initial evaluation, follow-up codes such as 99213, 99214 (office visit) and 90833, 90836 for psychotherapy with E&M.
- Ask how much your annual deductible and coinsurance
- Ask how many certified visits you have per calendar year
Step Two: Submit a detailed invoice or “superbill”, see how much reimbursement you receive, and read the enclosed E.O.B (Explanation of Benefits). Most health insurance companies are required by law either to pay the claim, or tell you why they are not going to pay within 30 days, you may call your health insurance company and ask about the “status” of your claim.
